Community School Self-Assessment

Directions: Use this Community School Self-assessment to identify areas to prioritize for you and your team’s learning. This self-assessment asks you to assess the four core components of a Community School:

  1. Community school coordinator role
  2. Assets and needs assessment
  3. Community school leadership team (often called an Advisory Council)
  4. Implementation plan 

This form will help you assess the current stage of each component of your community school, and is aligned with the Stages of Community School Development Framework.  As you complete the assessment, you may see that each core component may be at a different stage of development. This can help you identify the resources best suited to your community school, and the steps that you and your leadership team can take. After you complete the assessment, you will be emailed the self-assessment questions along with your responses.
